About The Position

The Nurse Navigator enhances the patient experience by providing a seamless navigation process from abnormal finding/diagnosis though the cancer continuum. Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate, and consistently manage patient care by educating the patient and serving as a point of contact for patients and families. Works with patients and families to answer questions, provide emotional support, and resolve any barriers, needs or issues related to the cycle of cancer care. Assesses patients' current and future needs, provides patient access to resources internal and external, facilitates distress screenings and patient needs assessments, and makes appropriate referrals. Establishes and maintains close collaboration with physicians and the cancer care team to ensure patient care and coordination using oncology nursing knowledge base, experience and expertise. Actively participates in multidisciplinary conference, providing patient follow up as needed. Provides patient education and symptom management support. Participates in community health promotion and awareness programs. Performs other duties as assigned.

Requirements

  • California active RN license required.
  • Associate of Science Degree in Nursing
  • Minimum of Three (3) years of oncology nursing experience required
  • Ability to problem solving, decision-making and critical thinking; excellent organizational, written and verbal communication and interpersonal skills
  • Work in a self directed environment and effectively handle multiple priorities.
  • Knowledge of the American College of Surgeons (ACoS) Commission on Cancer (CoC) standards is preferred.
  • Bi-lingual in English/Spanish is preferred.
  • Bilingual in Spanish
  • Prior experience planning, organizing, and completing individual treatment plans
  • Prior experience implementing professional and community based education programs
  • Proficient in Microsoft Office, Electronic Health Records (EHR), and related database programs
  • Knowledge of medical terminology, CPT coding, anatomy, histology, hematology and cancerrelated diagnostic procedures
  • Knowledge of the American College of Surgeons (ACoS) Commission on Cancer (CoC)standards
