Under the guidance of the Director of the Cancer Center, the RN Care Coordinator/Nurse Navigator is responsible for coordinating multidisciplinary clinic programs and ensuring seamless care throughout the entire treatment and surgical continuum for patients diagnosed with or undergoing therapy at the Medical Center. This role includes navigating patients through all phases of care, collecting and organizing essential data for weekly MDT meetings, securing missing medical records and imaging studies, and tracking patients and their treatment plans to optimize readiness for surgery. The RN will monitor patient adherence to prescribed plans, follow-up testing, and required visits to support quality of life, while also ensuring workflows align with current policies and educating the team on updates. This position plays a key role in maintaining CoC Accreditation in Colorectal and Pancreatic Cancer by collaborating closely with the Pancreatic TB Meeting providers and supporting regulatory compliance initiatives. This is an excellent opportunity for an experienced nurse who has a passion for oncology and wants to branch out into their nursing career. A caregiver in this role works days from 7:00 a.m. -- 5:00 p.m.

Requirements

Current licensure as a Registered Nurse (RN) in state of employment
Basic Life Support (BLS) Certification through the American Heart Association or American Red Cross
Any registered nurse or advanced practice nurse must obtain a cancer-specific certification or demonstrate ongoing qualifying education within the timeframe of the facilities accreditation cycle
Knowledge of oncology prevention diagnosis disease process and symptom management
Program completion in Professional Nursing
Five years of nursing experience including two years in oncology

Nice To Haves

Bachelor’s of Science in Nursing (BSN)
Experience in case management mental health program development and community health/home care
Oncology Certified Nurse (OCN)
Surgical experience (inpatient or outpatient)
Medical office experience

Responsibilities

Coordinate multidisciplinary clinic programs, program development and services.
Navigate the care continuum for patients by collaborating with the Medical Director and act as a "compass" for patients diagnosed and/or receiving therapy.
Ensure timely scheduling of diagnostic work-up, results reporting and communications between patients and the multidisciplinary team.
Promote the multidisciplinary team process and ensure patients receive necessary referrals and follow-up.
Provide efficient and effective care for patients and families and keep current with emerging technologies and treatment trends.
Coordinate cancer related patient/staff education programs, projects, services and groups.
Assist with community events, evaluate the results and continue outcomes research.
Compile and analyze Quality Improvement data and benchmarking.
Complete mandatory education and training to maintain organization and department specific competencies and requirements and certification/licensure.
