About The Position

The Registered Nurse (RN) - NurseConnect at Sauk Trails Clinic is responsible for providing triage, patient education, self-care advice, and general health information to members via telephone and online communication. This role operates under the supervision of the Clinic UC and Nurse Connect Manager, ensuring high-quality service and adherence to organizational policies while promoting the cooperative's mission and values.

Requirements

• Registered Nurse (RN) license in Wisconsin.
• Experience in triage and patient education.
• Strong communication skills, both verbal and written.
• Ability to work collaboratively in a team environment.
• Commitment to high-quality patient care and service.

Nice To Haves

• Experience with MyChart or similar online communication tools.
• Knowledge of community health resources.
• Bilingual skills.

Responsibilities

• Perform triage and assess patient needs over the phone and online.
• Provide patient education and self-care advice to GHC-SCW members.
• Deliver general health information to members via telephone or MyChart.
• Work under the direction and supervision of the Clinic UC and Nurse Connect Manager.
• Adhere to organizational and departmental policies and standards.
• Represent the organization positively in all interactions.
