The RN case manager assesses, plans, implements, coordinates and evaluates the plan of care in partnership with the family and other members of the health care team. Full time Monday-Friday (8:00 am – 4:30 pm) No evenings or weekends Onsite at the Milwaukee campus

Requirements

  • Associate Degree in Nursing – required
  • 2-3 years of experience with complex, chronic pediatric patients.
  • License RN-Registered Nurse (30) - State of Wisconsin
  • BLS-Basic Life Support Healthcare Provider - American Heart Association/American Red Cross/National Safety Council

Nice To Haves

  • Bachelor’s Degree in Nursing – preferred

Responsibilities
