About The Position

Regardless of role, we recognize that every member of our team is vital! he RN Navigator - Gastroenterology accurately and appropriately provides patient-centered care by means of health protection, promotion, optimization and prevention of illness or injury. This position performs both independent and delegated nursing functions including: assessment, nursing diagnosis, planning, implementation and evaluation. RN Navigator - Gastroenterology Overview Key responsibilities include but are not limited to : Establishes measurable and achievable short- and long-term goals for patients Implements nursing protocols based on the treatment plan, ensuring continuity of care for patient Provides education to patient and caregiver(s) Evaluation of the patient’s status and efficiency of the nursing care and modified care plan as needed Helps provide healthcare access through nurse visits, management of in-basket task (phone messages, MyChart communication, refill requests, results) Provides safe and timely telephonic triage for symptomatic callers, using system approved triage policies and protocols

Requirements

Current Minnesota RN Licensure
3+ years of nursing experience within specific patient population group.
Healthcare Provider Basic Life Support (BLS)

Responsibilities

Establishes measurable and achievable short- and long-term goals for patients
