Our pediatric specialty clinic is looking for an RN navigator to join the team! This role will help coordinate care to ensure families and patients have the treatment, education and resources necessary to meet their needs within the Pediatric Gastroenterology department. Our Pediatric Gastroenterology Clinic cares for patients with celiac disease, inflammatory bowel disease and eosinophilic esophagitis as well as a variety of other gastrointestinal concerns. In addition, our staff along with our pediatric gastroenterology providers, go to several outreach clinics throughout the region, allowing for increased access to the specialized care that is essential to their health and wellness. This is a great opportunity to work in a clinic setting with a variety of patients! We are open Monday-Friday 8:00am-5:00pm, no weekends or holidays. Requirements

  • Bachelor's degree in Nursing strongly preferred in Bemidji and Rapid City.
  • Required in Fargo and Sioux Falls (Sanford USD Medical Center and Clinics).
  • Required in Bismarck or enrolled and starting a Bachelor’s degree program within one year of hire with a completion date within three years upon date of hire into position.
  • Graduate from an accredited nursing program.
  • Minimum of two years’ clinical experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring.

Responsibilities

  • Integrates health care for patients/clients across the continuum of care.
  • Collaborates with healthcare team and other professionals to coordinate and ensure timely sequencing of care.
  • Supports patients/clients and other family members by removing barriers of care.
  • Seeks to evaluate outcomes based on an integration among established clinical, financial and utilization data.
  • Functions in structured and unstructured health care settings.
  • Utilizes independent nursing judgment when integrating health care.
  • Establishes and maintains good interpersonal relationships with those with whom has contact.
  • Monitors, evaluates and trends patient responses utilizing structured care methodologies.
  • Work closely with ancillary and support services.
  • Ability to solve problems, identify and implement process improvements when needed.
  • Facilitates groups, multidisciplinary teams, and implements best practices when necessary.
