Hurlbut Care Communities is seeking an experienced Full-Time RN MDS Coordinator to join our team! This position is integral to our dynamic group of professionals across our 11 Nursing Homes located in 4 counties. The primary purpose of this role is to provide Patient Centered Care, act as the patients' advocate, and coordinate the RAI process, ensuring the timeliness and completeness of the Minimum Data Set (MDS), Care Area Assessments (CAA's), and Interdisciplinary Care Plans. The ideal candidate must be devoted and compassionate about their chosen career and passionate about patient care. As the RN MDS Coordinator, you will serve as the patient advocate, bridging communication between patients, families, and facility staff. In this role, you will be responsible for managing the case mix with an emphasis on meeting or exceeding budget expectations. You will assess and evaluate residents for Medicare Part A coverage prior to and upon admission, schedule screenings, and determine coverage for all residents who may qualify for Medicare Part A. Additionally, you will complete and maintain MDS compliance per CMS guidelines, coordinate scheduling for annual, quarterly, and significant change MDS, and ensure timely submission to government agencies. You will also perform medical record reviews as requested by CMS/HMOs using their guidelines, oversee and assist nursing staff in maintaining MDS compliance, and train and update staff in MDS and care planning. Moreover, you will develop and update assigned policies and procedures, ensuring nursing department compliance with Medicare, Medicaid, and other insurance reimbursement regulations, including documentation. Monitoring and guiding the completion of assessments in a timely manner will also be part of your responsibilities, along with involvement and supervision of inputting MDS assessments, transmission to the state, and obtaining receipt of validation. You will schedule and facilitate care plan conferences and facilitate monthly Quality Management meetings, updating and revising patient care plans as needed.
