About The Position

Atrium Health Stanly Manor, located in Abermarle, NC, is an award-winning 90-bed nursing and rehabilitation facility providing short-term rehab, long-term care, palliative care, and hospice care. The facility is committed to enhancing the quality of life and well-being of its residents and has a MD or PA/NP on site 4-5 days a week. Stanly Manor received the 2024 Bronze—Commitment to Quality Award from the American Health Care Association and National Center for Assisted Living. The RN MDS Coordinator is responsible for collecting and analyzing resident health data per MDS-RAI/IRF-PAI guidelines, determining healthcare needs, and developing individualized comprehensive plans of care. This role ensures timely submission of MDS/IRF-PAI and care plan development according to facility policy and CMS regulations, practices ethically, delegates tasks to the interdisciplinary team, and demonstrates leadership in professional practice.

Requirements

  • Graduate from an accredited School of Nursing.
  • Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside or; if declaring a National License Compact (NLC) state as your primary state of residency, meet the licensure requirements in your home state; or for Non-National License Compact States, current RN License or temporary license as a Registered Nurse Petitioner required in the state where the RN works.
  • BLS required per policy guidelines.
  • 1 year experience with MDS/IRF-PAI and/or prospective payment system (PPS).
  • Additional education, training, certifications, or experience may be required within the department by the nurse leader.

Nice To Haves

  • BSN preferred.
  • Resident Assessment Coordinator-Certified strongly preferred.

Responsibilities

  • Collects data pertinent to the resident's/patient's health condition and required assessment elements per the MDS-RAI/IRF-PAI guidelines.
  • Analyzes the assessment data to determine the healthcare needs of the resident/patient.
  • Identifies expected outcomes and develops a comprehensive plan of care individualized to the overall healthcare needs of the resident.
  • Ensures MDS/IRF-PAI is submitted timely and plan of care is developed within required timeframes based on facility policy and CMS regulations.
  • Practices ethically.
  • Delegates elements of the MDS/IRF-PAI and care plan process to appropriate members of the interdisciplinary team.
  • Attains knowledge and competence that reflects nursing practice and skills relevant to the role of MDS/IRF-PAI Coordinator.
  • Demonstrates leadership in the professional practice setting and the profession.
  • Practices in an environmentally safe and health manner.
