About The Position

The RN/MDS Coordinator at The Heights Rehabilitation and Healthcare Center is responsible for conducting and coordinating the completion of Minimum Data Set (MDS) assessments in compliance with federal, state, and facility guidelines. This role is essential for ensuring regulatory compliance and supporting the delivery of quality care to residents, requiring a strong clinical background and excellent communication skills.

Requirements

  • Current Registered Nurse (RN) licensure in the state of Ohio.
  • Minimum of 2 years of nursing experience, preferably in a long-term care or skilled nursing facility.
  • Previous experience with MDS assessments and care planning is highly preferred.
  • Strong knowledge of federal and state regulations related to long-term care.
  • Excellent organizational, communication, and time-management skills.
  • Ability to work effectively as part of a team and independently.
  • Proficiency in using electronic medical records (EMR) systems and MDS software.

Responsibilities

  • Conduct MDS assessments for all residents in a timely and accurate manner, including admission, quarterly, annual, and significant change assessments.
  • Collaborate with interdisciplinary team members (nursing, therapy, social services, dietary, etc.) to gather and review information needed for the MDS process.
  • Develop and maintain care plans based on MDS findings, ensuring they are person-centered and aligned with regulatory requirements.
  • Ensure proper coding of all MDS data and submission to the appropriate agencies (CMS, state, etc.).
  • Review and analyze data for care planning and quality improvement purposes.
  • Monitor and track residents' clinical status and provide guidance to the nursing team regarding any changes in resident needs.
  • Educate staff on MDS processes, care planning, and compliance standards.
  • Maintain up-to-date knowledge of federal, state, and facility regulations regarding MDS assessments and documentation.
