Requirements

  • Must possess a Nursing Degree from an accredited college or university.
  • Must have a minimum of two (2) years of experience as a supervisor in a hospital, nursing care community, or other related health care facility.
  • Must have a minimum of six (6) months training experience in rehabilitative and restorative nursing practices.
  • Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices, procedures, regulations and guidelines governing long-term care.
  • Current State RN license.

Responsibilities

  • Conduct and coordinate the development and completion of resident assessments, in accordance with the requirements of State, Federal and Company guidelines.
  • Oversee the generation of Minimum Data Set (MDS) for each Medicare patient and electronic transmission of required data within time frame mandated by the State.
  • Coordinate the development and completion of the resident assessment (MDS) in accordance with current regulations and guidelines.
  • Implement CAAs and Triggers; conduct or coordinate the interview(s) of each resident for the resident's assessment.
  • Evaluate each resident's condition and pertinent medical data.
  • Develop and implement procedures with the Director, Clinical Services for arrival of newly admitted residents.
  • Ensure that all assessments are completed and transmitted in a timely manner.
  • Assist community directors and supervisors in scheduling the resident assessment/care plan meetings.
  • Contact and assist in scheduling participation by outside members of the care plan team, including the resident's representative and/or other interested family members.
  • Coordinate the development of a written plan of care (preliminary and comprehensive) for each resident.
  • Ensure that the care plan includes measurable objectives and timetables to meet the resident's needs.
  • Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
  • Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans.
  • Monitor the community's QI and QM reports to ensure that appropriate corrective action can be implemented when potential problems occur.
  • Assist the resident and Discharge Planning Coordinator in completing the care plan portion of the resident's discharge plan.
  • Participate in community surveys (inspections) made by authorized government agencies.
  • Provide leadership and participate in various committees including Interdisciplinary Care Plan Team, Policy Advisory, and Quality Assessment and Assurance.
  • Provide reports and assessment updates, as needed.
  • Develop and participate in the planning, conducting, and scheduling of timely in-service training classes.
  • Assist the In-service Director/Educator in developing any training activities needed.
  • Attend and participate in annual community in-service training programs as scheduled.
