Reporting to the VP of Quality, this individual is responsible for oversight of the Reston Hospital Center inpatient and outpatient patient safety program including the coordination of all relevant activities as per the requirements of CMS. This individual also collaborates with the risk management program. The Patient Safety Manager maintains records, reports, dashboards and prepares/delivers written and verbal reports. This position assists in ongoing staff and provider education related to quality and patient safety. The Patient Safety Manager provides departments with the most current performance improvement methods and tools; messaging trends in the data to departments and responding to serious patient safety events by conducting Root Cause or Apparent Cause analyses. Advances patient safety using systems thinking approach.

Requirements

  • Current VA Board of Nursing License for Registered Nurse.
  • BLS certification.
  • Certification in Patient Safety (CPPS) preferred.
  • Bachelor's Degree in Healthcare related field required, Master's Degree preferred.
  • 1+ years' experience in Patient Safety in an acute hospital environment preferred.
  • 3+ years bedside nursing experience in acute care setting strongly preferred.
  • Strong leadership qualities to include: follow through, motivation, prioritization, balancing multiple projects simultaneously.
  • Attention to detail and accuracy required.
  • Project management skills experience preferred.
  • Ability to work independently.
  • Proficient in MS Office, including Word, Excel, PowerPoint as well as data base and web-based program applications.
  • Excellent written and verbal communication skills.

Responsibilities

  • Chair of the Patient Safety & Quality Committee; responsible for setting meeting schedule, preparing material and agendas and timely follow up and follow through.
  • Identifies and mitigates Patient Safety risk through use of data, education, and Failure Mode and Effects Analysis (FMEA).
  • Supports Regulatory Compliance, in conjunction with the VP of Quality, from a Patient Safety perspective.
  • Works closely with department directors and leadership teams, patients, visitors, medical and allied health staff for consultation on Patient Safety.
  • Assists with minimizing risks by investigating and acting on all regulatory survey reports/recommendations related to Patient Safety.
  • Represents Patient Safety at departmental meetings.
  • Provides comprehensive patient safety education and orientation program to new hospital employees.
  • Increases Patient Safety Awareness and Practice among clinicians and staff.
  • Collaborates with VP of Quality and Compliance Officer in annual proactive safety risk assessment.
  • Conducts root cause analyses in response to unexpected occurrences which caused or may cause injury or harm.
  • Tracks and analyzes data for trending and develops appropriate action plans and patient safety strategies in conjunction with the VP of Quality.
  • Analyzes hospital acquired conditions, validation of compliance with bundles and determination of corrective action plans as necessary.
  • Presents information related to patient safety in a manner that meets the recipient's needs.
  • Assists with submitting data to state and federal regulatory agencies.
  • Collaborates with HCA legal team on cases involving Reston Hospital Center.
