RN Manager, Hospice Hospital Liaison
About The Position
Successful candidate will have experience as a Hospital Liaison, preferably in Hospice Leadership. Role will be based out of Concord but will cover Sutter Care at Home Footprint. Responsible for leading and overseeing the home health and hospice hospital liaison team to support seamless transitions from inpatient care to post-acute care services. Ensures that patient referrals, admissions, and discharges are coordinated efficiently and compassionately, while maintaining compliance with regulatory standards and organizational policies. Serves as a strategic partner to hospital leadership, physicians, and interdisciplinary teams, promoting Sutter Health as the provider of choice for post-acute care services.
Requirements
- Graduate of an accredited school of nursing
- RN-Registered Nurse of California
- BLS-Basic Life Support Healthcare Provider
- AUTO-Automobile Insurance
- DL-Valid Drivers License
- 5 years recent relevant experience
- Extensive experience in post-acute care
- Strong clinical knowledge of post-acute care
- Familiarity with reimbursement methods and regulatory requirements
- Excellent communication, leadership, and problem-solving skills
- Proficiency in EMR systems and documentation standards
- Ability to work in a fast-paced, emotionally demanding environment
