About The Position

EvergreenHealth is seeking an experienced RN to join the Mammography team. Job Summary: Responsible for the total direct nursing care of all patients within established standards of care, hospital and unit policies, procedures, and guidelines.

Requirements

Graduate of a nursing program (Diploma or Associate Degree in Nursing (ADN) required.
Licensed as a Registered Nurse in the State of Washington
Current Healthcare Provider BLS – American Heart Association b y start date.
Strong patient education and communication skills
Experience in care coordination and multidisciplinary team collaboration

Nice To Haves

BSN or MSN
Knowledge and experience with EPIC computerized charting.
2 years of clinical experience in breast health and/or oncology care navigation
Breast Navigation experience / BHCN Certification (Breast Health Clinical Navigation); or obtain w/in 2 years of start date.

Responsibilities

Provides direct patient care and func tions as a patient advocate.
Assesses the comprehensive nursing needs of an assigned group of patients.
Coordinates the patient and family/signific ant other education process.
Continuously plans, implements, and evaluates patient care, incorporating patient and family/signifi cant other into the process.
Participates with physician partners and other interdisciplinary team members to develop and implement patient and family/si gnificant other plan of care.
C ommunicates effectively all relevant information (e.g., patient information, safety issues, problems with equipment, quality issues, etc.) to immediat e supervisor, in timely way.
Communicates and documents changes in patient condition, on a timely basis, to charge nur se/supervisor and physician.
Performs other duties as assigned.
