About The Position

The RN/LVN Nurse Champion at Champions Healthcare is responsible for providing primary care to residents under the supervision of attending physicians. This role emphasizes assessment, illness prevention, and healthcare management, ensuring that residents' physical and psychosocial needs are met according to established medical practices and facility policies.

Requirements

  • High School Diploma or equivalent.
  • Graduate of an accredited Licensed Vocational or Practical Nurse program.
  • Active LVN or LPN license valid in the state of Texas.
  • Evidence of being free of tuberculosis infection upon hire.
  • Ability to read, write, speak, and understand English.
  • Ability to make independent decisions when necessary.
  • Knowledge of nursing and medical practices and regulations pertaining to long-term care.

Nice To Haves

  • Experience in long-term care settings.
  • Ability to handle difficult residents with patience and tact.

Responsibilities

  • Confer with the Medical Director and attending physician regarding specific residents assigned.
  • Consult with the physician concerning resident evaluation and assist the Director of Nursing Services in planning and developing nursing services.
  • Initiate requests for consultation or referral as needed.
  • Examine residents and their records to identify normal and abnormal findings.
  • Determine when to refer residents to a physician for further evaluation.
  • Perform administrative duties such as completing medical forms and charting.
  • Implement and maintain established policies, procedures, and quality assurance measures.
  • Make written and oral reports to the attending physician or Medical Director regarding resident care.
  • Participate in the planning and evaluation of nursing service policies and procedures.
  • Supervise and assist in managing the infection control program.
  • Assist in developing assessments of nursing needs for residents.
  • Administer medications and perform nursing procedures as required.
