Expect to spend about 50% of your time traveling across Cincinnati, Butler, Warren, Clinton, Hamilton, Clermont and surrounding areas. The Registered Nurse (RN) Care Manager or Licensed Social Worker (LSW/LISW) Care Manager works collaboratively as part of a care management team to onboard new families onto service and provide ongoing support to caregivers. This role is responsible for assessing consumer and caregiver needs, evaluating eligibility for service, and guiding prospective families through an efficient activation of services. The Nurse or Social Worker establishes a person-centric plan of care, completes an orientation to Careforth’s digital communication platform, and conducts regularly scheduled home visits. For an assigned caseload, this position coaches, advocates, and educates caregivers to assist with the daily health and well-being of their care recipients.

Requirements

  • Must hold an active Registered Nurse (RN) license and/or be a Licensed Social Worker (LSW/LISW) in the state of Ohio
  • Minimum of two (2) years of recent clinical experience providing direct care to elders or adults with disabilities
  • Ability and flexibility to travel within the assigned geographic area; a valid driver’s license and active car insurance are required
  • Approximately 50% travel, including on-site visits throughout Cincinnati, Butler, Warren, Clinton, Hamilton, Clermont, and surrounding areas
  • Experience with case management, care planning, and comprehensive assessments, including eldercare, individuals with disabilities, and patients with complex medical needs in a healthcare setting
  • Ability to work effectively in a fast-paced environment while meeting departmental goals and KPIs
  • Self-motivated with the ability to influence others and drive results
  • Technically proficient and comfortable teaching others; experience with common software and web-based applications (e.g., case management systems, Microsoft Office)
  • Additional state-specific qualifications may be required

Responsibilities

  • Manages the enrollment process of prospective families and educates them on qualifications as it relates to the consumer, caregiver, and home; assesses consumer and caregiver eligibility for services
  • Helps guide consumers with applications for Medicaid, waiver programs, and other state benefits
  • Conducts initial in-person clinical assessments and coordinates service activation, ensuring that all required forms and standard procedures are followed
  • Assists with orientation training for each caregiver to facilitate a proper onboarding to Careforth’s HIPAA secure technology platform; ensures caregiver understands how to successfully submit electronic daily communications
  • Works collaboratively to develop each member’s plan of care and review on an ongoing basis
  • Provide ongoing coaching to drive effective utilization of established best practices, encourage caregiver education, and connect families to community resources
  • Reviews the caregiver notes daily and appropriately addresses changes in the consumer’s condition or environment
  • Completes documentation corresponding with each on-site visit or encounter
  • Monitors the health status of all members to ensure that all needs and programmatic requirements are properly delivered (i.e. incident management)
  • Collaborates and communicates regularly with clinical team, agency and support services, primary care physician, and other medical providers to ensure timely and appropriate delivery of consumer services
  • Ensures continuous compliance with all regulatory and accreditation agency standards as well as company policies and procedures
  • Plans and implements discharges and transitions from the program
  • Perform other duties as assigned

  • The pay range for this position is $61,248- $82,281.The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
