About The Position

The Registered Nurse (RN) and Licensed Practical Nurse (LPN) work collaboratively to deliver high-quality, patient-centered nursing care using the nursing process of assessment, planning, implementation, and evaluation. They are integral members of the multidisciplinary clinical team, committed to the mission and philosophy of Hebrew SeniorLife (HSL), ensuring patients' dignity, autonomy, and quality of life. RNs maintain accountability for comprehensive assessments and clinical oversight, while LPNs contribute under the supervision of an RN, focusing on ongoing care delivery and supporting care plans. Both roles actively support culture change initiatives and serve as role models and mentors within their units.

Requirements

  • Graduate of an accredited nursing school
  • Valid Massachusetts nursing license in good standing (RN or LPN)
  • Previous clinical experience preferred
  • Strong interpersonal, communication, and leadership skills
  • Flexibility, professionalism, and adaptability to changing priorities
  • Computer literacy and proficiency with electronic medical records
  • Excellent English communication skills (written and verbal)

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred

Responsibilities

  • Deliver nursing care consistent with professional standards, policies, and HSL core competencies
  • Collaborate with interdisciplinary team (MDs, NPs, social workers, therapists, dietitians, etc.) to meet patient needs
  • Implement evidence-based interventions to achieve optimal outcomes
  • Participate in care planning, promoting autonomy and individual choice
  • Document accurately in electronic medical records
  • Identify and utilize appropriate resources to ensure safe, effective care
  • Maintain compliance with all policies, regulations, and HIPAA
  • Participate in quality assurance, performance improvement initiatives, and shared governance
  • Respond to and report safety concerns or incidents
  • Actively engage in unit activities and assist with tasks (e.g., meal service)
  • Serve as role models and mentors to PCAs and other clinical staff
  • Conduct comprehensive assessments and lead care planning (RN-specific)
  • Provide clinical oversight to LPNs and PCAs (RN-specific)
  • Revise care plans based on assessment data (RN-specific)
  • Co-sign LPN assessments and initial care plans (RN-specific)
  • Initiate incident, DPH, and concern resolution reports (RN-specific)
  • Lead rounds (wound care, physician visits) (RN-specific)
  • May assume charge nurse responsibilities, including delegating assignments and supervising shift operations (RN-specific)
  • Support RN-led assessments and contribute to care implementation (LPN-specific)
  • Supervise and delegate to PCAs (LPN-specific)
  • Ensure care plan fidelity and update documentation under RN oversight (LPN-specific)
  • Assist with reports (DPH, incidents) as directed (LPN-specific)
  • May assume charge nurse duties under RN supervision, including providing shift reports and monitoring documentation (LPN-specific)
