RN, LPN or OT Case Manager

JOB PURPOSE: Obtain post admission authorizations Act as the liaison between the physicians, hospitals and Insurance Case Managers Assist in the development of Care Plans for medically complex residents Manage and coordinate caseload upon admission, throughout stay/discharge Timely communication with updating external physicians/Case Mng Communicate patient care needs with the IDT Support patients and their families Attend weekly Medicare Meetings Evaluate for denials and assist with appeals Coordinate discharges including transport, meds, and referrals for home care. Follow up with patient and/or family post discharge Other duties as assigned
Requirements
- Three years clinical case management experience with at least one in long term care required.
- Advanced communication, problem solving and interpersonal skills.
- Knowledge of Medicare and Medicaid regulations in long term care required.
