Requirements

  • Valid Indiana RN or LPN License
  • 2+ years of skilled nursing experience
  • Matrixcare experience
  • Experience level sufficient to limit clinical orientation to 2 shifts
  • On-call nurses must be available at least one day per month for an 8-hour period.
  • Nurses do not create their schedule, but may pick up open shifts up to one week in advance or respond to a request to cover openings.
  • Provide staffing coordinator with each month’s availability no later than the 15 th of the month prior.
  • Respond to staffing calls within 60 minutes of contact.
  • Remain within a 30-minute radius of the facility during on-call hours.
  • Maintain phone accessibility at all times during on-call shifts.
  • If no shifts are picked up within 30 days, the employee will be removed from payroll.

Responsibilities

  • Provide professional nursing services including resident assessments, vital signs, and medication administration
  • Assist with restorative and rehabilitative care and daily living instruction
  • Conduct regular rounds and communicate observations
  • Document and report unusual or abnormal findings, care plan changes, and follow-up needs
  • Supervise nursing personnel and provide job-specific guidance
  • Communicate effectively with residents, families, and staff
