About The Position

The position is for an LPN or RN at Fairlawn Haven in Archbold, OH, offering contingent opportunities with varying shifts, including holiday and weekend rotations. The LPN will focus on monitoring the health status of resident care plans throughout the Care Center, assisting in various neighborhoods such as Rehab, Assisted Living, Special Care, and Long-Term Care. The candidate will also assist and lead STNA staff. The ideal candidate should possess good assessment skills, the ability to work effectively in a changing environment, be a team player, have a positive attitude, and be self-motivated.

Requirements

• Graduate of an accredited Nursing Program
• Current LPN nursing license in good standing with the State licensing board
• Minimum of one (1) year Long Term Care experience preferred
• Ability to maintain confidentiality and honor Resident’s Rights
• Excellent attendance and work history

Responsibilities

• Monitor health status of resident care plans
• Assist in Rehab, Assisted Living, Special Care, and Long-Term Care neighborhoods
• Lead STNA staff
• Communicate effectively with personnel, residents, family members, visitors, and government agencies
