About The Position
The position involves monitoring the Electronic Health Record nurse pool throughout the day and responding in a timely manner. The role requires triaging incoming calls, messages, and In-Basket tasks, prioritizing urgent matters, and scheduling appointments as needed. The nurse will exercise nursing judgment and standard triage guidelines to ensure the health needs and concerns of patients are met. Assessing complex patient needs is essential to ensure patients are transitioned to the appropriate level of care. The nurse will demonstrate a systematic approach to collect objective and subjective data and assess patient responses. Additionally, the role includes providing clinical support to the team, answering phone calls and electronic messages promptly, and greeting callers in a friendly and respectful manner. The nurse will document all calls and actions taken into the patient's medical record timely and completely, educate patients about community services, and collaborate with the interdisciplinary care team to avoid interruptions in service delivery. Other responsibilities include obtaining vital signs, assisting clinicians with procedures, resolving patient concerns, participating in meetings and trainings, maintaining patient rights, providing clerical support, and floating to other practices when required.
