About The Position

Our sub-acute floor is a 59 bed Medicare unit that has residents with multiple clinical issues including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac related issues needing monitoring. Nurses work closely with a variety of health care professionals including doctors, social work, dieticians, PT/OT, hospice, and spiritual support. Our sub-acute unit offers a well-rounded opportunity for learning, professional growth and personal satisfaction. Our Chronic Care floor is a 60 bed Medicare/Rehabilitation and long-term care unit and is a great place to learn basic med-surg, I.V. therapy & wound treatments nursing skills. Nurses on this unit along with host of healthcare professionals work to provide and implement the best treatment plan for residents. This fast paced and busy unit provide a great atmosphere for PT/OT residents to be motivated and reach their goals. Our Dementia Care floor is a 60 bed Medicare/Medicaid/Dementia unit. We provide care to residents with a variety of diagnoses. The dementia care is where we create a fullness of life through the joy of relationships, the art of caring and the spirit of living. This unit also works closely with a multidisciplinary team meets and discusses the resident’s plan of care.

Requirements

  • Associate degree ADN or equivalent.
  • Michigan LPN license upon hire.
  • BLS – AHA or ARC upon hire.

  • Experience in Skilled Nursing in Long term care.

  • Delivers nursing care responsibly and accountably as described professionally by the American Nurses Association (ANA) and legally by the State of Michigan.
  • Demonstrates competency through the critical thinking model known as the nursing process, including assessment, diagnosis, outcomes identification, planning, implementation, and evaluation.
  • Upholds the standards of professional performance as described by the ANA, including ethical practice, culturally congruent practice, communication, collaboration, leadership, education, evidence-based practice and research, quality of practice, professional practice evaluation, resource utilization and environmental health.
  • Strives to achieve optimal outcomes.
