T ransitional Case Manager-RN or Licensed Social Worker Full Time The Parker Jewish Institute for Health Care and Rehabilitation is pleased to announce a new 30-day care transition program for patients at high-risk of readmission post-discharge from inpatient care. This interdisciplinary, proactive care model addresses the needs of patients at highest risk for poor outcomes post-discharge. The program's goals are to reduce avoidable readmissions, and improve patient care and well-being by connecting patients to primary care and needed social services. For this program, we are seeking a Licensed Social Worker or RN Case Manager to serve as key members of the interdisciplinary care transitions team to provide support, follow up and access to resources for patients in the community. Working on site at Parker, the team will work closely with our interdisciplinary team to design and implement the operational details of this intervention including following the discharge plan, provide early engagement, patient education and coaching, as well as assist patients in overcoming potential barriers to care, along with thorough ongoing patient follow-up.
