About The Position

It's more than a career, it's a calling MO-SSM Health at Home Wentzville Worker Type: Regular Job Highlights: We are looking for someone who is compassionate, detail-orientated and able to work independently while following medical protocols. The selected RN will provide care to patients in their homes. Duties include; medication management, Wound Care, Patient Monitoring, Personal care assistance, Education and support and documentation. -RNs typically see 5 patients per day -Schedule: 8 to 430 Every third weekend in rotation and typically have 2 to 3 on-call shifts in a six week period. -.70/mile We have an amazing group of nurses that are dedicated to teamwork, work well with each other, help each other out and back each other up. Job Summary: Provides leadership and coordination of clinician/patient staffing and/or scheduling while ensuring implementation of clinical objectives and quality patient care to patients within a home setting or an SSM hospice house location.

Requirements

• Graduate of accredited school of nursing or education equivalency for licensing
• One year of registered nurse experience
• Proof of motor vehicle liability insurance that meets minimum state requirements upon hire and upon request.
• Constant use of speech to share information through oral communication.
• Constant standing and walking.
• Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
• Frequent sitting, reaching and keyboard use/data entry.
• Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
• Frequent use of smell to detect/recognize odors.
• Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
• Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
• Occasional lifting/moving of patients.
• Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
• Frequent driving.
• Rare crawling and running.

Responsibilities

• Assigns, leads, educates and monitors nursing and support staff during assigned shift.
• Contributes to or performs performance evaluations of staff.
• Functions in the role of supervisor or branch manager in branch leader absence and takes a lead role in process improvement responsibilities.
• Assists branch leaders in preparing clinical reports and operational duties.
• Assesses daily patient care needs and develops and distributes patient care assignments, ensuring that population appropriate patient care is given.
• Performs direct patient care as needed.
• Coaches staff on effective and efficient case management & collaborates with other health care providers to deliver results for positive clinical & financial outcomes.
• Collaborates with advanced practice providers to ensure quality outcomes.
• Evaluates the effectiveness of patient care and identifies any issues that affect desired patient outcomes.
• Responds to complaints about patient care and manages through established channels.
• Performs ongoing chart review on case load with attention to: a) Accurateness of 485, recertification, verbal orders for changes in Plan of Care. b) Assuring every visit performed is within assigned frequency. c) Assuring overall documentation supports coverage of service as defined by regulation or payor.
• May provide input for hiring, training, directing, developing and evaluating staff.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.
