About The Position

The Intake & Referral Coordinator is a member of the Intake Team and the Home Health Senior RN Team. Responsible for the collection and verification of intake and referral information necessary to screen and process patients appropriate for admission to the home health program. Responsible for oversight of Insurance Preauthorization, Precertification, and Re-authorization. Coordinates the entire intake process.

Requirements

  • Current Alabama RN license.
  • A minimum of 2 years working as a Registered Nurse required.
  • Must be highly organized and have demonstrated leadership ability.
  • Good communication skills and a record of on the job exemplary behavior are necessary.
  • Must be able to read, write legibly, speak, and comprehend English.
  • Must have current BLS certification or be able to obtain within 60 days of employment.

Nice To Haves

  • Prefer knowledge of Home Health business operations.

Responsibilities

  • Establishes and maintains positive working relationships within the referral community.
  • Screens patients for home health needs. Communicates with referral sources as it relates to home health services.
  • Ensures all information needed to expedite a smooth transition to home health is appropriately documented and effectively communicated.
  • Continually assesses manual and electronic process and functions to identify ways to improve the intake process.
  • Maintains accurate record of daily referrals, including referrals not accepted and the reason/s any referral is not accepted.
  • Monitors referrals that become non-admit for trends and improvements in the intake process.
  • Communicates with referral sources and insurances case managers and obtains authorizations if required and follows up on all ongoing services for continues authorizations with RN Care Managers.
  • Ensures that all information required for insurance preauthorization and precertification is forwarded to the appropriate payer to expedite accurate billing of claims.
  • As a member of the Home Health Leadership Team participates in the Administrative RN On-call rotation.
  • Coordinates the entire intake process.
  • Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation.
  • Performs compliance requirements as outlined in the Employee Handbook
  • Must adhere to the DCH Mission, Vision, and Values including creating positive relationships with patients/families, coworkers, colleagues and with self.
  • Performs essential job functions in a manner that ensures the safety of patients, visitors and employees.
  • Identifies and reduces unsafe practices that may result in harm to patients, visitors and employees.
  • Recognizes and takes appropriate action to reduce risks and hazards to promote safety for patients, visitors and employees.
  • Requires use of electronic mail, time and attendance software, learning management software and intranet.
  • Must adhere to all DCH Health System policies and procedures.
  • All other duties as assigned.
