About The Position

The position is for a part-time Registered Nurse in Case Management at Advocate Health, specifically within the AMC Summit department. The role requires 28 hours of work per week, primarily during the first shift from Monday to Friday, with a Saturday rotation. The candidate will be responsible for conducting assessments, establishing care plans, and coordinating discharge planning services to ensure patient satisfaction and compliance with regulatory standards. The position is on-site, with no travel required, and is expected to start after the current employee retires on 9/5/2025.

Requirements

  • Registered Nurse License issued by the state of practice.
  • Bachelor’s Degree in Nursing.
  • 2 years of clinical nursing experience.
  • 3 years of inpatient experience preferred.
  • Ability to prioritize and organize work.
  • Effective communication skills.
  • Utilization of critical thinking and timely decision making.
  • Ability to navigate the Electronic Health Record.
  • Basic utilization of MS Office products.
  • Knowledge of Medicare A and B guidelines.
  • Knowledge of Managed Care program requirements.

Responsibilities

  • Conduct complete assessments and establish appropriate care plans.
  • Collaborate with patients, families, and healthcare teams to achieve care goals.
  • Utilize patient/family strengths in the problem-solving process.
  • Provide continuity of care and discharge planning services.
  • Manage family dynamics and crisis situations effectively.
  • Document discharge planning interventions and utilization review activities.
  • Communicate effectively with the healthcare team.
  • Work in partnership with Social Work and support personnel.
  • Serve as an active member of the Outcome Facilitation Team.
  • Collaborate with managers and physicians on care issues.
  • Complete Utilization Management activities as required.
  • Develop relationships with community-based agencies.
