ASSESSMENT: Completes patient assessments competently. Assesses for and identifies needs of unit-specific patient population: Assesses for and identifies any physical, emotional, psychosocial, spiritual, educational, discharge and cultural needs. PLANNING: Completes an individualized plan of care based on assessment of the patient. Coordinates with other disciplines when indicated. Prioritizes care needs and sets measurable goals and time frames. Includes patient and family in the planning. IMPLEMENTATION: Implements interventions based on nursing and medical plan of care. Manages patient population, diagnosis and age specific needs through appropriate adaptation of interventions. Appropriately utilizes equipment and supplies (including following established policies, procedures and protocols governing the transport, handling and storage of intravenous fluids). EVALUATION: After implementation, reassesses the patient to verify effectiveness of interventions. Updates the plan of care to reflect current care needs. CRITICAL THINKING: Demonstrates the ability to recognize and adapt to changes in patients condition (i.e.. LOC change, vital signs, test results)in a timely and clinically appropriate manner. Uses SBAR to notify physicians and other providers of clinical findings. Follow policies and procedures for contacting physician regarding critical values. Uses appropriate resources and follows chain of command. Recognizes limits of scope of practice. Seeks multidisciplinary collaboration. Integrates & interprets multiple sources of data to make appropriate clinical decisions based on patient needs & desired outcomes. Takes responsibility and accountability for patient/family satisfaction by using the nursing bundle. Demonstrates AIDET Principles. Demonstrates 8 behaviors of Hourly Rounding and uses the care board when applicable. Assesses patient/family personal needs and provides Individualized Care. Conducts Discharge Phone calls when assigned. Involves patient/family in Shift Bedside Report with oncoming/offgoing nurse. Initiates service recovery when indicated. Uses proper etiquette in person, on the phone & in emails. Assist others by escorting/directing visitors. CONTINUOUS IMPROVEMENT: Identifies, makes suggestions for and/or initiates process improvement with new and innovative ways to improves practice and care delivery. COMPLIANCE WITH MEDICATION & PATIENT SAFETY STANDARDS (INCLUDING NATIONAL PATIENT SAFETY GOALS): Meets safety standards. Provides for patient safety according to professional and regulatory standards. Writes legibly. Minimizes override of medications. Identifies and documents allergies, lactation status, height and weight on all patients. Restricts use of unapproved abbreviations. Documents readback of verbal orders, phone orders and critical test results. Double checks medications when required. Performs essential elements of Medication Reconciliation process. Properly identifies patients using two identifiers. Demonstrates safe hand-off communication using SBAR.
