RN- Hospital Care Manager- Up to $$74.30/ hour Interim Health Care Staffing is currently hiring hospital RN Care Managers for immediate needs in premier local hospital! Schedule: Full-Time, M-F 8am - 4:30pm Pay rate : Up to $74.20 depending on experience Position Responsibilities Include: Conducts thorough and accurate psycho-social assessments of patients in an inpatient hospital unit in a timely manner with a patient centered approach. Actively participates on interdisciplinary team to develop and implement care plans during hospitalization. Establishes and implements care plans to ensure safety, continuity of care, appropriate care coordination and support post hospitalization. Uses knowledge of community resources, healthcare funding, along with county, state, and federal regulations in making appropriate and safe discharge plan. Communicates promptly, clearly, respectfully and effectively with care team, patient and support system. Coordinates and attends care conferences as applicable. Documents work clearly, concisely and consistently in a timely manner. Works closely with the Social Worker throughout hospitalization and during discharge planning. Required Skills/Qualifications: An active RN license in the states of MN. BSN is preferred but not required. years of current RN experience is preferred. One year of hospital Case Management experience is preferred. Current BLS/ CPR Ability to pass a criminal background check. Requirements

An active RN license in the states of MN.
Current BLS/ CPR
Ability to pass a criminal background check.

Nice To Haves

BSN is preferred but not required.
years of current RN experience is preferred.
One year of hospital Case Management experience is preferred.
