Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not a cure, and on quality of life, not duration. As an RN this team, you will collaborate with physicians and fellow caregivers to support patients within our Care Center. In this role, you can make a difference in how they view and adapt to an uncertain future, no matter what that may hold. Additionally, this position offers a flexible schedule and autonomy to work independently, allowing you to maintain a healthy work-life balance while continuing to serve as an emotional ally and comfort. A caregiver in this role will work 7:00am to 7:30pm. Caregivers will also work every other weekend and 2 holidays a year.

Requirements

  • Graduate of an accredited school of professional nursing
  • Current licensure as a Registered Nurse (RN)
  • Current Basic Life Support (BLS) certification through the American Heart Association (AHA)
  • Valid State Driver’s License (VDL) and proof of $100,000/$300,000 automobile
  • One year of current clinical experience as an RN
  • One year of experience in an acute medical-surgical or critical care setting

Nice To Haves

  • Bachelors of Science in Nursing (BSN)
  • Hospice and Palliative Care Nurse Certification

Responsibilities

  • Provide supervision, assessment, planning and direct patient care to hospice patients.
  • Perform accurate and complete admissions assessments and re-assessments.
  • Assess patients for safety risks, abuse and neglect.
  • Assess, document and manage patient’s pain.
  • Prioritize and document patient problems.
  • Administer medications and treatments safely and accurately with a focus on patient comfort.
  • Coordinate care for managing a caseload of patients.
  • Document changes in patient condition and effectiveness and timeliness of care/interventions.
  • Evaluate, update and revise plans of care.
  • Assess educational needs of patients, caregivers and family while considering cultural issues and other special challenges.
  • Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs.
