As an On-Call Hospice RN, you will be a vital part of a dedicated team that delivers patient-centered care in the place patients call home. We are seeking a skilled and empathetic On-Call Registered Nurse to provide after-hours hospice care. In this role, you will respond to urgent patient needs, provide symptom management, offer emotional support to families, and ensure our patients experience dignity, peace, and comfort during their end-of-life journey. Requirements
- Current RN license in the state of Louisiana
- Minimum of 1-2 years of hospice experience
- Strong clinical judgment and the ability to work independently in a home setting
- Compassionate approach to end-of-life care and support for families
- CPR certification, valid driver's license, reliable transportation, and auto insurance
- Knowledge of state and federal home health regulations. Clinical proficiency in nursing, strong communication skills and compassion
Responsibilities
- Respond promptly and professionally to after-hours calls and patient visits
- Provide expert pain and symptom management in accordance with individualized care plans
- Support families and caregivers with compassion, education, and reassurance
- Collaborate with the interdisciplinary team to ensure continuity of care
- Maintain accurate documentation and uphold compliance with all clinical and regulatory standards
