About The Position

As a Hospice Supervisor, you will provide leadership in promoting and maintaining standards for high-quality care by all members of the patient team. You will direct office clinical activities including patient assessments, development of plans of care, and assist with the determination of service levels. Your role will involve implementing, coordinating, maintaining, and evaluating plans of care, ensuring quality service, and participating in care coordination to facilitate proper communication with team members, patients, physicians, referral sources, and payer sources. You will also review clinical records and documentation to ensure regulatory and agency compliance, participate in quarterly clinical record review programs, and provide back-up support to staff. Additionally, you will conduct on-site visits for patient care and/or supervision of staff members as needed, prepare and deliver employee performance appraisals, and participate in the development of administrative policies and procedures relating to the agency.

Requirements

  • Current license as a Registered Nurse in Pennsylvania
  • Extensive Hospice experience
  • BLS Certification
  • Exceptional leadership, interpersonal, and communication skills
  • Must have current driver's license
  • Ability to travel to case assignments
  • Excellent assessment skills, documentation skills and ability to meet deadlines

Responsibilities

  • Provide leadership in promoting and maintaining standards for high-quality care by all members of the patient team
  • Direct office clinical activities including patient assessments and development of plans of care
  • Assist with determination of service level and implement, coordinate, maintain, and evaluate plans of care
  • Ensure quality service and participate in care coordination to facilitate proper communication with team members, patients, physicians, referral sources, and payer sources
  • Review clinical records and documentation to ensure regulatory and agency compliance
  • Participate in quarterly clinical record review program and performance improvement activities
  • Provide back up support to staff; supervise and teach to improve practice with the agency
  • Conduct on-site visits for patient care and/or supervision of staff members as needed
  • Prepare, review, and deliver employee performance appraisals
  • Perform employee counseling when needed; identify performance deficiencies/areas for growth and provide necessary training, coaching, and/or tools
  • Participate in the development of administrative policies and procedures relating to the agency
  • Discuss operational issues and assist in planning the overall development and administration of the agency
  • Keep staff informed of additions or changes in policies
  • Maintain current practice knowledge through internal and external training programs/seminars
  • Attend at least one in-service annually
