As an RN on the Home Care Team, you will deliver the best possible care to patients in the comfort of their own homes. In this role, you will provide assessments, complex treatments and education to help avoid readmissions to the acute care setting and ensure comfort. Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Cleveland Clinic’s hospice team includes physicians certified in hospice and palliative medicine, nurses, social workers, chaplains, home health aides and volunteers. Join our team to work together supporting patients at home or in a skilled nursing facility or hospital. This is a PRN role. A caregiver in this position works between 11:00am and 11:30pm. Requirements include every other weekend and 1 winter/1 summer holiday on rotation every 3 years.

Requirements

  • Graduate from an accredited school of Professional Nursing
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA)
  • One year of clinical experience as an RN in an acute medical/surgical setting
  • Valid Driver’s License (VDL) in Ohio and proof of automobile insurance with $100,000/$300,000 coverage

Nice To Haves

  • Bachelor’s of Science in Nursing (BSN)
  • Infusion therapy experience

Responsibilities

  • Provide direct nursing care in accordance with established healthcare organization policies, procedures, and protocols.
  • Implement and monitor patient care plans.
  • Provide supervision to LPNs, Home Care Aides and families/caregivers involved in the care of the patient.
  • Perform accurate and complete admission assessments and reassessments within a specified time frame.
  • Assess patient for safety risks, abuse and neglect.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Assess and document patients' pain using appropriate pain scale and manage patients' pain then.
  • Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Escalate change in patient condition to patient’s physician as appropriate.
