Job Responsibilities includes for a RN Hospice Float Nurse includes: RN Hospice Float Nurse needed for Upper Burlington & Mercer Counties. Weekend requirements 6 per year and holiday requirements 1 per year.

Requirements

  • R.N. required; BSN Degree preferred
  • NJ RN license required, CHPN or OCN preferred, BLS training
  • NJ driver’s license and insurance coverage required
  • Minimum of one year nursing experience - medical/surgical, oncology, geriatric or related experience, hospice/home care experience preferred.

Responsibilities

  • Performs initial and on-going patient assessments for comfort and palliative patient care
  • Obtain patient’s history on admission
  • Establishes and implements an individualized plan of care for each patient
  • Assesses the need for patient and family education and provides education as needed
  • Obtains appropriate physician orders to achieve symptom management
  • Provides safe and appropriate skilled nursing care in accordance with protocols, policies, procedures and physician orders
  • Manages and administers medications according to standards
  • Documents in EMR in an accurate, concise and timely manner per standards
  • Triages and arranges home visit schedule, demonstrating efficiency in time and mile management
  • Meets or exceeds departmental productivity standards, including number of visits per week.
  • Assigns patient care to other team members as appropriate (LPNs and CHHAs) and observes and evaluates the effective ness of their work
  • Makes referrals to additional IDT members and collaborates with social workers related to the involvement of additional IDT members
  • Fosters IDT collaboration by initiating communication to address PFC needs/updates of plan of care
  • Anticipates potential problems and establishes preventative measures
  • Performs effectively in emergency situations
  • Follows OSHA and agency regulations in the use of standard precautions, the management of controlled substances and biological
  • Provide basic personal care and ADL’s when appropriate.
