Requirements

• Active RN license in the state of practice.
• Minimum of 1–2 years of nursing experience, preferably in hospice, palliative care, or home health.
• Strong assessment, clinical, and communication skills.
• Knowledge of hospice regulations, standards, and care planning.

Responsibilities

• Assess, plan, implement, and evaluate individualized hospice care for patients.
• Coordinate care with physicians, nurses, social workers, and other healthcare professionals.
• Provide education and support to patients and their families regarding disease progression and end -of -life care.
• Ensure documentation complies with hospice standards and regulatory requirements.
• Monitor patient conditions and adjust care plans as necessary.
• Participate in interdisciplinary team meetings and contribute to quality improvement initiatives.
