About The Position

This position is based in Lakeland, FL and requires daily travel to Valencia Hills and surrounding areas. It is full-time, M-Friday, 8am - 5pm. Compassionate Care Hospice, an Amedisys Company, is seeking a dedicated RN Case Manager Nurse to join our exceptional team in Lakeland. Salary Range: $80,000–$86,000 annually, with additional bonus opportunities. Final compensation may vary based on job-related knowledge, skills, and current years of direct acute care experience as a Registered Nurse. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience. Current RN license, specific to the Florida state you are assigned to work. 1+ year(s) of clinical RN experience in acute healthcare setting(s) - REQUIRED. HCHB experience - preferred. Current CPR certification.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
