Our Outpatient Hospice and Palliative Care team is seeking an experienced Hospice RN to facilitate Hospice and Palliative Care referrals from all sources and obtains reviews & coordinates the necessary information to complete the clinical assessment of the patient for the correct program & correct LOC within that program. The position assumes dual accountability for Palliative Care and Hospice referrals.
Requirements
- Associate Degree in Nursing
- State of New Mexico or Compact State Nursing License
- BLS certification REQUIRED at time of hire
- ACLS
- PALS
- 1 year of hospice experience required
- Current drivers license and auto insurance.
- Basic typing competency.
Nice To Haves
- 1 year of acute care experience strongly preferred.
Responsibilities
- Provides palliative care & hospice services based upon patient assessment, including direct patient care.
- In all care, utilizes nursing assessment skills to determine presence of symptoms and response to interventions for those symptoms.
- Assessments include a review of the full spectrum of patient needs, including an understanding of how the physical, psychological/social (behavioral), functional, financial, resource and spiritual needs of patients factor into their condition.
- Serves as a consultant to discuss goals of care with the patient/family and communicates those goals to the appropriate care team.
- In addition, reviews referrals, and delegates the referral to the appropriate hospice/palliative care staff based on patient needs and staff availability.
- Performs Hospice consult reviews and coordinates a clinical assessment, determines if patient is appropriate for hospice, communicates with the patient/family about hospice philosophy, criteria, and site of care.
- Assumes role of hospice RN with hospital hospice patient, including development of hospice plan of care and subsequent rounding, if necessary.
- Acts as a clinical resource for all members of the hospice /palliative team.
- Develops and maintains a working knowledge of all services available within the Home and Transitions Service Line.
- Completes comprehensive and accurate documentation timely and consistently that clearly reflects interventions performed.
- In all roles, performs as a member of an interdisciplinary team.
- Assists with orientation and mentoring of new team members as appropriate.
- Serves as a resource person & professional role model.
