About The Position
The Home Health RN provides professional nursing services to patients in their homes, focusing on quality care, patient safety, and improved health outcomes. This role supports patients across the continuum of care, from post-acute recovery to chronic disease management.
Requirements
- Current, unrestricted RN license
- Graduate of an accredited nursing program
- Valid CPR/BLS certification
- At least one year of clinical nursing experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Ability to manage workload independently
Responsibilities
- Conduct comprehensive patient evaluations and reassessments during home visits
- Establish and manage individualized care plans in collaboration with physicians
- Administer medications, treatments, and specialized nursing procedures
- Monitor patient conditions and respond to changes with appropriate interventions
- Record clinical notes and maintain accurate, timely documentation
- Educate patients and families on treatment plans, medications, and self-care strategies
- Coordinate with healthcare team members to ensure continuity and effectiveness of care
- Adhere to all regulatory, safety, and organizational standards
