Cleveland Clinic currently serves 13 counties in the greater Cleveland and Akron areas. The caregiver in this role will serve patients in the West area (Amherst, Avon, Brook Park, Cleveland, Elyria, Lakewood, Lorain, Parma, Strongsville, Vermillion and Westfield). As a Homecare Float Registered Nurse, you will be responsible for providing high quality nursing care to patients in the comfort of their own homes. As an RN, you will provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. After seeing patients, Homecare RNs work remotely to complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules. If there are no visits, RNs monitor quality through a variety of means, such as chart audits and supervisory visits. RNs are reimbursed for mileage (patient to patient) and are eligible for student loan repayment. A caregiver in this position works 10:00am to 10:30pm on 3 12-hr shifts. No weekend, holiday or on-call requirements.

Requirements

Graduate from an accredited school of professional nursing
Current state licensure as a Registered Nurse (RN)
Basic Life Support certification through the American Heart Association or American Red Cross
One year of current clinical experience as an RN
One year of experience in an acute medical/surgical or homecare setting
Valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage

Responsibilities

Provide direct nursing care in accordance with established policies, procedures and protocols of the agency, taking visits on the uncovered list, late admissions and other patient visits as needed.
Participate in and support quality improvement projects.
Participate in departments and committees as assigned.
Supervise starts of care, resumptions of care and routine visits as requested.
Perform accurate and complete admission assessments and reassessments within a specified time frame.
Assess patient for safety risks, abuse and neglect.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Assess and document patients' pain using appropriate pain scale and manage patients' pain then.
Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team.
Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
Escalate change in patient condition to patient’s physician as appropriate.
