The caregiver in this role will serve patients in the Eastern Cuyahoga and Lake County area (including but not limited to Ashtabula, Beachwood, Cleveland, Euclid, Willoughby and Mentor). As a PRN Homecare RN, you will be responsible for providing high quality nursing care to patients in the comfort of their own homes. As an RN in this role, you will provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. Familiarity with complex medical treatments from experience working in home care, med/surg, ICU, or the emergency department is helpful. However, Cleveland Clinic Home Care provides a generous in-depth orientation that is tailored to each individual and their experience level. A caregiver in this position works varying shifts from 8:00am – 5:00pm. This is a PRN position that requires you to work at least 48 hours every six weeks (including one weekend shift, if needed). You must also complete eight weeks of full-time orientation Monday – Friday from 8:00am – 5:00pm.

Requirements

  • Graduate from an accredited school of professional nursing.
  • Current state licensure as a Registered Nurse (RN).
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • One year of clinical experience as an RN in an acute medical/surgical or homecare setting.
  • Valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage.

Nice To Haves

  • Bachelor of science in nursing (BSN).
  • Infusion therapy experience.
  • Homecare experience

Responsibilities

  • Provide direct nursing care in accordance with established healthcare organization policies, procedures, and protocols.
  • Implement and monitor patient care plans.
  • Provide supervision to LPNs, home care aides and families/caregivers involved in the care of the patient.
  • Perform accurate and complete admission assessments and reassessments within a specified time frame.
  • Assess patient for safety risks, abuse and neglect.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Assess and document patients' pain using appropriate pain scale and manage patients' pain then.
  • Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Escalate change in patient condition to patient’s physician as appropriate.
