About The Position
Amedisys is seeking a highly skilled and compassionate registered nurse (RN) for a rewarding career in homecare as a case manager. As a home health RN, you will play a crucial role in providing patient-centered care, helping patients navigate the complexities of their healthcare needs in the comfort of their own homes. In this role, you will perform patient assessments, collaborate with the care team to develop and implement individualized care plans, and promote patient health and independence through education and rehabilitative measures. You will also supervise LPNs and HHAs, ensure timely and accurate documentation, and communicate patient progress to the clinical manager and care team. Additionally, you will participate in clinical development and continuing education programs, and may be required to perform on-call responsibilities as assigned.
