The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide professional nursing services to patients/residents at home. The nursing process includes the documentation of human responses to actual or potential health problems in the home health environment. This practice includes dependent, interdependent and independent functions. Collaborates with patient/resident and family, other inter-professional colleagues, such as physicians, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned setting. Cares for patients/residents in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up for patients/residents in the home setting.

Requirements

  • Graduate from a nationally accredited nursing program preferred.
  • Bachelor's degree in nursing preferred.
  • Minimum of one year of professional nursing experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing.
  • Obtain and maintain required department specific competencies and certifications.
  • Basic Life Support (BLS) required.
  • Certificate for Outcome and Assessment Information Set (OASIS) Specialist - Clinical (COS-C) preferred.

Nice To Haves

  • Experience in home health care.
  • Multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Valid driver's license may be required depending on location.

Responsibilities
