About The Position

The Home Health Nurse, RN at CenterWell provides skilled nursing care to patients in their homes, enabling them to maintain their independence while receiving necessary medical attention. Requirements

  • Diploma, Associate, or Bachelor Degree in Nursing.
  • Minimum of one year nursing experience preferred.
  • Skilled Home Health (Medicare OASIS) experience required.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.

Responsibilities

  • Provide admission, case management, and follow-up skilled nursing visits for home health patients.
  • Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
  • Confer with physicians to develop the initial treatment plan based on orders and patient assessments.
  • Provide hands-on care, management, and evaluation of care plans, teaching patients in accordance with physician orders.
  • Revise care plans in consultation with physicians based on ongoing assessments and regulatory requirements.
  • Coordinate care among various healthcare personnel, including therapists and home health aides.
  • Report patient care, condition, and progress to the physician and Clinical Manager continuously.
  • Implement patient care plans in conjunction with patients and families to achieve optimal resolutions of needs/problems.
  • Supervise the work of Home Health Aides and provide written personal care instructions reflecting the current care plan.
  • Monitor documentation completion by home health aides as part of supervisory responsibilities.
  • Discharge patients after consultation with the physician and Clinical Manager, preparing necessary clinical documentation.
  • Prepare medical documentation on all patients, including case conferences and care plan changes.
  • Prepare visit reports, summarize patient records, and confer with other healthcare disciplines.
