About The Position

The After Hours Home Health Nurse assumes primary responsibility for patient care delivery; received referrals and communicates with physician’s offices, staff and patients on medical orders received whenever the Home Health Office is closed.

Requirements

  • Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.
  • Valid multi-state or State of Oklahoma Registered Nurse License.
  • A valid drivers license, Motor Vehicle Report and proof of vehicle liability insurance in the amount required by SFHS guidelines will be required.
  • 1 year of related nursing experience.
  • Good interpersonal and communication skills.
  • Knowledge of Medicare Home Health Regulations.
  • Reliable motorized transportation as needed for home visits.

Responsibilities

  • Utilizes the nursing process to determine eligibility for home health, assess health care needs, initiates patient teaching, and begins discharge planning.
  • Informs/educates patient/family/caregiver and obtains written consents for mandated government and agency specific forms.
  • Performs SOC initial assessment and initiates plan of care.
  • Assess/initiates eligibility for additional disciplines and community services in a timely manner.
  • Utilizes resources in planning and pressing patient education.
  • Delivers nursing care with consideration of culture and physiological factors.
  • Organizes priorities and performs nursing skills appropriately in accordance with initial identified needs.
  • Anticipates and delivers patient care according to assessed needs.
  • Demonstrates professional judgment in response to changes in patient/agency situation.
  • Instructs in emergency, safety and initial home management of assessed needs.
  • Completes required SOC documentation established by agency guidelines.
  • Documents the steps required for a complete and accurate SOC record.
  • Records data concisely, consistently, accurately and legibly (when appropriate) that reflects patients status and identified skilled needs on admission.
  • Records accurate OASIS data collection via patient demonstration or clinical observation.
  • Utilizes correct medical terminology, abbreviations and ICD 9 codes by established agency guidelines.
  • Supports agency/hospital goals through Performance Improvement/Quality Assurance and education activities to promote quality patient outcomes.
