About the Home Health/Hospice Team Our Home Health and Hospice teams pride themselves on providing top quality care in the home to those individuals requiring additional healthcare services or during their end-of-life journey. Mission for Position The Home Health/Hospice RN enhances quality of life for home care and hospice patients by delivering skilled nursing care, promoting comfort and dignity, and partnering with patients, families, and the care team to support holistic care. Position Type This is a part-time weekday position working 40 hours per pay period (0.5FTE), in both home care and hospice settings. Hours are 8:00am - 4:30pm, with one weekend rotation every 12th week and one holiday rotation each year.

Requirements

  • Current Registered Nurse (RN) license in the State of Minnesota.
  • Valid Minnesota driver’s license and current auto insurance for travel between patient homes.

Nice To Haves

  • Experience in home health or hospice nursing preferred.
  • Strong patient assessment, care planning, and clinical decision-making skills.
  • Ability to educate patients and families on disease processes, symptom management, and end-of-life care.
  • Comfortable working independently in home environments while collaborating with an interdisciplinary care team.
  • Strong organizational, communication, and documentation skills, including proficiency with electronic medical records.
  • Knowledge of home care and hospice regulations and best practices.

Responsibilities

  • Assess patient needs and complete admission assessments within required timeframes.
  • Develop, implement, and update individualized nursing care plans in collaboration with physicians and the interdisciplinary care team.
  • Provide skilled nursing care, patient education, and guidance to patients and families in the home or hospice setting.
  • Monitor patient conditions, document care in the electronic health record, and report significant changes to the physician.
  • Ensure Home Health Aides and LPNs adhere to care plans and ensure quality care.
  • Coordinate care with therapists, social workers, and community resources to support patient outcomes.
  • Ensure compliance with home care and hospice regulations, policies, and documentation standards.
  • Participate in team meetings, discharge planning, and continuous improvement of home-based services.
