Requirements

  • Current RN license in the state of practice.
  • Minimum of 2 years nursing experience, preferably in home health or community health.
  • Strong assessment, critical thinking, and clinical skills.
  • Excellent communication skills both verbal and written.
  • Ability to work independently and as part of a multidisciplinary team.
  • Familiarity with electronic medical records (EMR) systems.
  • Compassionate and patient-centered approach to care.

Responsibilities

  • Assess and evaluate patient needs to develop and implement individualized care plans.
  • Coordinate and oversee the delivery of patient care in a home setting.
  • Serve as a primary contact for patients and their families regarding care and treatment.
  • Collaborate with healthcare teams to ensure comprehensive patient care.
  • Monitor and document patient progress and outcomes in compliance with policies.
  • Conduct patient education regarding their health, care plans, and disease management.
  • Manage clinical documentation and ensure adherence to regulatory standards.
