  • Conduct comprehensive assessments of patients' health and develop individualized care plans.
  • Coordinate and manage patient care in collaboration with multidisciplinary teams.
  • Monitor patient progress and make necessary adjustments to care plans as required.
  • Educate patients and their families on treatment options and care needs.
  • Provide direct nursing care to patients in their home settings as needed.
  • Document patient condition and care plans accurately in electronic health records.
  • Ensure compliance with healthcare regulations and organizational policies.
  • Current RN licensure in the state of practice.
  • Bachelor's degree in Nursing preferred; relevant experience may substitute.
  • Minimum of 2 years of nursing experience, preferably in home health or hospice settings.
  • Strong communication and interpersonal skills to interact with patients and families empathetically.
  • Ability to work independently and make sound clinical decisions.
  • Familiarity with electronic health records and medical documentation standards.
  • Basic Life Support (BLS) certification required, Advanced Cardiac Life Support (ACLS) preferred.
  • Advanced Cardiac Life Support (ACLS) preferred
