As a home care registered nurse (RN), the role involves ensuring patient comfort and building meaningful connections with patients and their families. This position carries the significant responsibility of guiding patients and families through the end-of-life journey, with a strong emphasis on maintaining trust. VITAS nurses are respected team members, acting as both caregivers and hospice educators, and are granted the autonomy to make bedside care decisions to provide immediate comfort. A key aspect of the role is to help patients and their loved ones understand and embrace the palliative, comfort-focused nature of hospice care.

Requirements

  • Currently licensed to practice nursing in the state where the VITAS program is located
  • A minimum of two years of nursing experience
  • Reliable transportation
  • Commitment, Compassion, and a Can-do Attitude
  • A minimum of two years of nursing experience in hospice, home health, or community health in the last five years

Responsibilities

  • Ensure hospice patients are comfortable
  • Forge compassionate, meaningful connections with patients and their families
  • Guide patients and their loved ones through the end-of-life journey
  • Maintain trust with patients and families
  • Serve as a caregiver and hospice educator
  • Exercise autonomy in making care decisions at the bedside
  • Educate patients and loved ones on the palliative nature of hospice
  • Educate patients and loved ones on the benefits of comfort-focused end-of-life care
  • Conduct 4-6 patient visits daily in various home settings (private residences, assisted living/skilled nursing facilities)
  • Attend weekly interdisciplinary team meetings at the local program office
  • Prioritize patient welfare in all actions
  • Extend comfort and dignity to all people at the end of their life’s journey
