Dedicated to The Care of Elders: PACE ( P rogram of A ll-Inclusive C are for the E lderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible. Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services. Join our team of leaders to begin a rewarding career! POSITION SUMMARY: The Home Care Supervisor will assess, develop, implement and coordinate home care services for On Lok participants. The Home Care Supervisor is responsible for the management of daily home care operations including service provision, service delivery, and quality of care, staffing and personnel support.

Requirements

  • Graduate from an accredited college or university with either an Associate or Baccalaureate degree in nursing and current California Registered Nurse license.
  • Minimum three years' experience as an RN serving the geriatric or frail population required.
  • Experience in leadership roles such as Charge Nurse and/or Management
  • Two years of acute care experience (ICU, ER or Critical Care) and/or Home Health/Public Health experience.
  • Recent experience in a role with supervision requirements.
  • Active RN License
  • BLS Certification
  • California Public Health Nurse certificate.
  • Excellent verbal/written communication skills.
  • Ability to work as part of multidisciplinary team.
  • Moderate computer literacy and knowledge of Microsoft Office Suite.
  • Demonstrated ability to maintain an upbeat, positive approach to coordination, reinforce positive behaviors and create a highly effective team.

Responsibilities

  • CLINICAL PRACTICE Provides skilled observation, nursing assessment, development and implementation of appropriate home care treatment plans for participants with home care needs.
  • Teaches and counsels participants and family members regarding participants' care and safety related to their home environment and needs at home.
  • Provides skilled nursing care in a home/community setting as warranted by participant condition.
  • Triages participants according to needs and acuity of symptoms.
  • Provides medication management to include assessment for medication compliance and medication education.
  • Teach, supervise, and counsel the participant, On Lok Caregivers, and family members regarding home nursing care.
  • Act as a nursing consultant to Interdisciplinary Team.
  • Acts as a liaison and advocate to provide education to participants and families.
  • SUPERVISION & TRAINING Provides support, supervision and training to geriatric aides and home care nursing staff in carrying out the home care treatment plan.
  • Teaches and counsels participants and family members regarding participant's care and safety.
  • Provides education, training and mentoring to staff within the home care department as well as new hire staff.
  • Acts as a resource, preceptor and mentor to new employees, students and other team members.
  • Schedules and facilitates monthly home care meetings.
  • Supports and enhances the leadership skills of RNs, LVNs, and CGs.
  • Identifies and supports home care team professional and clinical developmental needs.
  • Acts as a resource, preceptor and mentor to new employees, students and other team members by demonstrating knowledge of specific conditions commonly seen in participant population.
  • Collaborates with Program Manager and Nurse Manager as issues or concerns arise in the community.
  • Demonstrates ability to identify and resolve problems in collaboration with home care team, program management and nursing leadership team.
  • Actively participates/leads in activities related to home care workflows and relevant improvement issues.
  • Participates in daily leadership huddles to ensure open, accurate, and timely communication within the home care team and among all disciplines/departments.
  • Delegates related tasks to appropriate staff.
  • Partners with medical leadership to ensure efficient home care operations.
  • Conducts Supervisory responsibilities, which include performance evaluations for directly supervised staff, payroll approval and staff scheduling.
  • Collaborates with other supervisors/managers and departments.
  • Other reasonably related duties as assigned.
  • COMPLIANCE Upholds the standards of the California Nurse Practice Act, American Nurses Association Standards of Nursing Practice and Code of Ethics for Nurses.
  • Practices nursing care in accordance to HIPAA standards and On Lok policies and procedures.
  • Acts as a mandated reporter as defined by On Lok's policies and guidelines and those requirements as defined by CA Board of Registered Nurses.
  • SAFETY MONITORING, TRAINING & REPORTING Adheres to infection control guidelines as stated by On Lok's policies, procedures and guidelines.
  • Actively participates in process improvement initiatives supporting On Lok's misson and goals.
  • Actively participates in improving quality metrics for the assigned center and meeting organizational goals.
  • Adheres to standards, policies and procedures of On Lok.
  • Adheres to safety policies and guidelines of On Lok.
  • STRATEGIC PLANNING & LEADERSHIP Provides home care representation and perspective in organizational strategy and planning by attending various committee meetings.
  • Advances the goals and objectives of the organization by developing supportive department specific performance goals, implementing quality improvement plans , participates in establishing and monitoring financial targets and works collaboratively with other supervisors/managers on issues related to quality programming efficiency and cost effectiveness.
