About The Position

The community health nurse is a professional nurse knowledgeable in community health nursing who provides nursing care to patients in the community setting. This position involves completing weekly schedules, making scheduled home visits, and prioritizing nursing visits according to client acuity and the organization's emergency preparedness plan. The nurse will develop individualized and holistic plans of care for patients, perform assessments, document visits, manage medications, and coordinate care with other disciplines. The role also emphasizes safety awareness, professional development, and leadership.

Requirements

• Graduate of an accredited School of Nursing.
• Bachelor of Science degree in nursing from a college/university accredited by the NLN is preferred.
• One to two (1-2) years of medical/surgical nursing experience is preferred.
• Ability to write, speak, and understand English.
• Basic Nursing competencies.

Responsibilities

• Completes weekly schedule and makes scheduled home visits per assignment.
• Develops an individualized and holistic plan of care for each patient in collaboration with the patient/family/caregiver.
• Performs admission physical and psychosocial assessments to determine appropriate plans of care.
• Demonstrates skills and judgment necessary to implement the plan of care and updates it as goals are met.
• Documents discharge outcomes and prepares patients for discharge.
• Completes medication reconciliation every visit and teaches medication management to patients/family members.
• Coordinates the plan of care for each patient and communicates with all disciplines involved.
• Fosters a 'Culture of Safety' and complies with infection control guidelines.
• Participates in opportunities for professional development and leadership.
• Demonstrates advanced clinical skills and incorporates evidence-based practice into clinical practice.
