About The Position

The RN Home Care & Hospice Case Manager position at Mid Michigan Home Health & Hospice involves providing comprehensive nursing care to patients in their homes. Requirements

  • Must possess a current, valid RN license in the state of practice.
  • Minimum of 1 year of home care experience.
  • BLS certification.

Responsibilities

  • Assess the patient's physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs.
  • Review patient history and physical assessment with patient/family and assure completion within appropriate time frame.
  • Review available information obtained by other health care team members and diagnostics/laboratory data, reporting abnormal results to the physician(s).
  • Complete assessments and reassessments according to patient need and policy guidelines.
  • Formulate a plan of care, including the discharge plan, utilizing assessment data and input from the patient, family, and health team.
  • Initiate a plan of care based on patient-specific needs and the medical plan of care.
  • Collaborate with multidisciplinary team members in planning care.
  • Review and revise the plan of care to reflect changing patient needs based on evaluations.
  • Implement the plan of care through direct patient care and supervision of the healthcare team.
  • Promote continuity of care by communicating patient status to other caregivers.
  • Evaluate the patient's and family's response to care and teaching, revising the plan of care as needed.
  • Document assessment, planning, implementation, and evaluation in the patient record accurately and timely.
