Responsibilities
Performs an individualized client-centered assessment and prioritizing data collection based on the patient's immediate condition or needs within a specified timeframe per policy.
Conducts ongoing assessments as determined by the patient's condition, per policy, procedures or protocols and reprioritizes care accordingly.
Documents patient assessment findings both physical and psychosocial responses to nursing intervention and work towards problem resolution
Develops a plan of care that is individualized for the patient population served, reflecting collaboration with the other members of the healthcare team.
Performs therapeutic nursing interventions as determined by the patient's individualized plan of care.
Provides individualized patient/family education specific to the patient population served
Interprets, documents and monitors patient's cardiac rhythm per policy. Report changes in cardiac rhythm to MD.
Identifies early signs of deterioration and responds to emergency situations. Demonstrates proficiency in performing life saving measures according to adult resuscitation protocols.
Reports critical test results to MD including laboratory, radiology, POCT.
Initiate and implement applicable core measures and protocols.
Adheres to infection prevention protocols including CLABSI and CAUTI.
Completes tasks assigned by charge RN or nurse manager.
Adheres HIPPA rules and regulations and current patient safety standards.
Ensures proper discharge instructions and medication reconciliation including follow-up referral
Assists with bedside interventional procedures
Performs other duties as assigned within the scope of practice.
